Short answer
Starting an NEMT business in California means forming a business entity, obtaining commercial auto insurance that explicitly covers passenger transportation, and enrolling as a Medi-Cal provider through the Department of Health Care Services — which uses the PAVE portal for provider enrolment. California also draws a formal distinction between non-emergency medical transportation and non-medical transportation, and which one you intend to provide changes your requirements, your vehicle and your rates.
California formally separates two categories that many other states blur, and getting this wrong at the planning stage wastes months.

Medi-Cal provider enrolment runs through the Department of Health Care Services, which uses the PAVE portal for provider applications. Managed-care plans operate across much of the state as well, so being enrolled and being contracted with the plans serving your county are two separate things.
